1. Being inclusive and consultative of all partners

There is a wide range of member agencies and stakeholders who have varying levels of involvement in the partnership, depending on their organisational goals and the work of the PCP.  It wont always be possible to work directly with all partners all the time but the partnership will be relevant to the core business of all member agencies in at least 1 key deliverable stated in this Plan.

2. Connecting better with senior management

This Plan seeks to formalise communication and reporting with Chief Executive Officer’s of member agencies.  This aims to ensure senior manager’s have a clearer understanding of the PCP and how the partnership can support their core business.

3. Meeting member expectations with few resources

The PCP can not be everything to all people.  Its level of resourcing does not support staff to coordinate grass-roots community development initiatives nor to provide intensive organisational support to all agencies.  This Plan identifies the work the partnership will do, including a focus on population health; supporting major catchment-wide initiatives; training and development; and phased intensive agency support.

4. Recognising and celebrating achievement

Agency staff have a wealth of experience and commendable commitment to improving the health of their communities.  Recognising this is an important way of celebrating achievements and supporting staff

5. Addressing underlying barriers to good health, including: transport, education, employment, social isolation, housing and family/parenting.

Our Location

The Southern Grampians and Glenelg PCP catchment is located in South West Victoria, 300km west of Melbourne.

SGG PCP Catchment

Our Partners

The SGG PCP is an unincorporated joint venture consisting of agencies which have signed the agreement.  Membership is open to any incorporated body which provides primary care services in the catchment area subject to the approval of members.

The SGG PCP agrees that other parties may become involved in the processes of partnership development, services coordination and service planning via the signing of a Service Linkage Protocol (SLP).  A SLP is a formal statement setting out the purposes, goals, responsibilities and agreed working relationships between each of the parties who agree to its terms.  It is not legally or financially binding.  Parties to the SLP have no voting rights but shall have the right to receive information, reports and may participate in the primary care development via membership on working groups as determined by the Executive Committee.
