to support member agencies to increase consumer management of their chronic illness by service integration of chronic disease management
Action will
- ensure options for self management are available to people with chronic illness across the PCP catchment
- assist member agencies in workforce development
- support member agencies to meet the needs of clients with chronic illness through comprehensive assessment and care planning
- link closely with the PCP’s work in Integrated Health Promotion and Service Coordination
SGG PCP recognises the impact of chronic illness on the carers, friends and families of people living with chronic illness. The SGG PCP recognises that the enhanced management of chronic illness needs to take a holistic approach to care, involving the range of disciplines in the consumer’s care and recognising that the consumer will need to take responsibility for managing their condition. Options for self management, including ways to increase their level of activity and healthy eating are required in our small rural centres.
