SGG Drug & Alcohol Plan

The Southern Grampians & Glenelg Drug & Alcohol Action Plan was endorsed by both Councils at their May meetings and was endorsed by the PCP’s Integrated Planning Subcommittee and Executive In June. The Plan was developed by the PCP with our partner agencies.  The Action Plan is an integrated, multi-agency plan and adopts a preventative and harm minimisation approach, using a health promotion framework. It has a clear focus on alcohol as the main priority, with some attention to tobacco use.
The PCP’s Integrated Planning Subcommittee will oversee the implementation of the Plan by partners agencies over the next two years.

SGG PCP Action Plan 2009-2012

In addition to the Strategic Plan, an Action Plan has been developed to further outline our specific activities for the upcoming three years, with a key focus on the current twelve month period. The Action Plan will be used for all of our planning and reporting on activities for all staff. This will then provide the Executive Committee with a quarterly snapshot of where we are. Furthermore, the Action Plan will form the basis for the Annual Report, together with some additional case study information

Chronic Conditions Consumer Journeys Report

We have launched ‘Chronic Conditions Consumer Journeys Project: Rural Consumers’ Experiences of Chronic Conditions living in Portland & District’. This work was conducted to inform planning for the SGGPCP’s ICDM activities in Portland and district as well as Portland District Health’s Early Intervention in Chronic Disease Initiative. 23 local community members diagnosed with a Chronic Disease, including 3 carers, participated in the project which involved interviews and focus groups to gather information about their experiences accessing and utilising local health services. Participants were deidentified in the report. As a result of these discussions a number of recurrent issues were identified and recommendations were made so as to improve experiences and outcomes for community members living with a chronic condition.

The Southern Grampians & Glenelg Primary Care Partnership 2009-2012 Strategic Plan has been endorsed by the Executive Committee. The Strategic Plan has been developed following extensive consultation with partners and key stakeholders. The end result is a concise document that clearly outlines our direction for the upcoming three years. As a diverse community organisation, we have been very conscious of the language used throughout the plan, ensuring that it is accessible to a wide audience.
