Goal

• to support agencies to increase consumer access to services by standard and integrated approaches to initial contact, needs identification assessment, referral and care planning.

Action will:

• assist agencies to use a standard approach to service coordination, particularly those new to service coordination, including disability, mental health and family services.
• support further development of e-referral
• continue to improve access for clients by increasing pathways through system re-design
• continue to support a regional approach to service coordination in partnership with SW PCP and the SW Alliance of Rural Health (SWARH)
